 I am glad that you have decided to take the next step to better health by making your first appointment. Please fill out the form below and I will re-contact you to schedule a time. Then download the New Patient Intake and Consent to Treat forms. Please fill this out and bring the forms with you to your first appointment. This will save about 15 minutes.

Please note the Cancellation Policy at Health Rising Wellness.
- Your appointment time is reserved specifically for you. In the event of a missed appointment or an appointment cancelled with less than 24 hours notice, you may be charged the full cost of the missed appointment.-

All of my current patients may reach me for IN OFFICE TREATMENTS OR telehealth consultations and herbal refills at the number listed below.

I am providing telehealth consultations for ALL of your health questions. There are many methods which may provide you with health assistance that do not require hands-on, in person service. Herbs, nutrition, lifestyle counseling, self care methods, to name a few. I will mail supplies to you as appropriate for your concern

Those of you who are current patients watch for email updates weekly.

Those who wish to see me when I reopen, go to the Appointments page on this website and send me your contact information. I will contact you asap. I will then add you to the email list for future announcements.
